Incompetent leaders who don't understand their customers. Leaders are in charge of people who do work that they (leaders) have never done for themselves. In the cases that "experts" are hired... they come from large companies that had a lot more resources and tools. They are too far removed from the "grunt" and as a result are more concerned with keeping their jobs/pay than actually fixing anything with strong decision making.
